- Submit ONE COMPLAINT FORM for each respondent.
- Include sections for privacy and terms policy with confidentiality agreements:

- • Ensure data is visible only to authorized personnel.
- • Display policies and guidelines, including complaint and response procedures.
- • Include a disclaimer reassuring users of confidentiality.
